MyQualityCareNetwork is a network infrastructure company. We are not a HIPAA covered entity. We do not provide medical care, operate a health plan, or process claims. Our role is to organize provider information, support credentialing readiness, and facilitate plan access discussions with Medicare Advantage organizations.

This website and its public-facing forms are not intended to collect Protected Health Information (PHI) about patients or health plan members. Providers, payers, and visitors should not submit member-identifiable clinical information, claims data, or other PHI through this site, contact forms, email, or general portal messaging.

Information submitted by providers, such as name, NPI, practice location, specialty, license information, and credentialing documentation, is used to support recruitment, credentialing readiness, and network development activities. This information is treated as business and professional information rather than PHI, although we apply appropriate safeguards.

If a specific engagement with a health plan or provider organization requires the exchange of PHI, that exchange will occur under a separately executed Business Associate Agreement (BAA) and through approved channels, not through this public website.

We apply reasonable administrative, technical, and physical safeguards to information submitted to us, including access controls, encryption in transit, audit logging, and role-based access within our portals.
